Registration for Pharmacy of Your Choice scheme in GozoThe Department of Pharmacy of Your Choice [POYC] wishes to advise all patients who are entitled to free medicines from the Government and who usually collect their medicines from the Pharmacy at the Gozo General Hospital, can now collect a Registration Form to benefit from the Scheme of Pharmacy of Your Choice from the POYC Office, at the Gozo General Hospital or the pharmacies listed below.

Due to the overwhelming response, the registration period for the POYC in Gozo, has now been changed to Monday, May the 31st.

Registration is important and patients must take and present the following:

1. Their Identity Card

2. The Yellow Card-s / the Card and the pink Form / Card and White for controlled drugs [as applicable]

3. The permit entitling them to drugs on the approval of the Department of Health [MEU]

The Registration Form must be filled as follows:

Section [A] – From the patient

Section [B] – From the pharmacist in charge / a pharmacy from which the patient has chosen to register

Section [C] – From the physician (private or health centre) or physicians of the Pharmacy at Gozo General Hospital.

The completed Registration Form, should be taken back to the pharmacy of the patients choice, along with the Registration Form, the patient should also provide copies of all permits and cards entitling them to free drugs..

It is IMPORTANT that registration should be made ONLY with the official Registration Form provided by the Department of Pharmacy of Your Choice. Applications will be accepted until 22 May 2010.
