Parliamentary Secretary for the Elderly and Community Care, Mario Galea today visited the Commcare services. During the visit he said that in 2012, the CommCare Unit received 8446 requests for domiciliary nursing care, of which 3173 were new referrals and 5273 were follow ups.

CommCare Unit consisits of a multidisciplinary team whose aim is to co-ordinate Community Care services around patient’s needs in a personal centred holistic manner with a case management approach.

Mario Galea expalined that this service is aimed for clients who are house bound. Cases are referred by doctors, hospitals, professionals and other entities. The concerned client or his family members can phone up and demand the service themselves.

The aim of the unit is principally to stipulate the eligibility of the service, to identify the needs and to guide the individual.

This unit is also responsible to authorise the nursing services at the individual’s residence which are provided by MMDNA according to the plan of action stipulated by the Unit. During the year 2012, MMDNA organiseed nearly 500,000 visits.

Parliamentary Secretary Galea mentioned the Outreach service; a new programme provided by the CommCare Team. This is meant for clients who owing to illnesses lost their capability to care for themselves independently.

CommCare Team consists of nurses, physiotherapists, occupational therapists, social worker and carers. The team visits the person in his or her home to identify the needs of the client, thus a cure programme can be set.

The team members work together with the patient and the respective family for a stipulated period of time so that the programme is successful.
