A new Case Management service with the aim of providing more support to people living in the community has been implemented.
People who cannot leave their homes and have chronic diseases and complex conditions; people who are experiencing difficulty in coping with their condition; and those who are frequently admitted to the hospital, among others, will be eligible for the Case Manager service.
This service by Active Ageing and Care in the Community (AACC) will include a Case Manager who will be coordinating and providing care focused on the client’s needs in the community.
This was announced during a news conference attended by the Minister for Active Ageing Jo Etienne Abela.
Through this service there will be a reduction in the amount of people admitted to hospital or residential homes, he said.
It was explained that they would even be able to assist the client to benefit from other services, such as Respite, and those provided by other entities including the Primary Care Department.
In his address, Minister Jo Etienne Abela supported the importance of this service which will continue to ensure the best care in the community.
He also pointed out that complex cases that fall under the mentioned criteria, are discussed between the management and the professional, in order to start a Case Management service on the case, after the first assessment is carried out at the client’s home.
AACC CEO Dr. Renzo De Gabriele explained that doctors and/or professionals can refer clients who need a holistic assessment in the community by the CommCare team through the Active Ageing and Care application in the Community and mark “Domiciliary Nursing” or by calling 22589393.
